Please use this form to submit a ticket. Alternatively, you can send an email to support@ticket.seas.gwu.edu with the same information.

Please enter an email we can use to contact you. Please use your gw email.
Please select your department.
Please enter your phone number.
Please include the building code with your room number. (i.e. TOMP, SEH,....)
Please enter your campus address.
Please select the campus where you are located.
If your ticket is in regards to a machine with a SEAS Tag please include it (i.e. SEAS10001). SEAS Tag's are blue and white stickers with a 5 or 6 digit number found on machines.
Please provide a short summary of the issue.
Please describe your issue in as much detail as possible.
